Senior Celebration 2023 Ticket Price:

$175 if paid on April 9 and after

Permission slip and waiver form

By registering below, I hereby give my permission for my son/daughter to attend the Notre Dame Prep Senior Celebration starting at 8 p.m. on May 21, 2023 and ending at midnight on May 21, 2023. I understand that my son/daughter must be checked in by 8:20 p.m. and will not be allowed to leave Legacy 925 until the event ends at midnight.

I acknowledge and represent that I am the parent or legal guardian of the above named student and that I have read and understand the information regarding the 2023 Senior Celebration. I also understand that the games and athletic events available may pose certain risks to the participants. I therefore, waive and hold Notre Dame Preparatory School and Marist Academy, the Senior Celebration Committee and volunteers, the Board of Trustees, harmless from liability relating to the use of such games and activities. I also understand that if I wish to revoke this permission, I must do so in writing within 48 hours prior to the event.

You will not receive a ticket for admission, and tickets will not be sold at the door. Names will be posted on the NDP office window of paid students and updated as needed.

Please do not bring outside beverages. Sorry, there will be no refunds.
